I am working as an independent contractor FNP on weekdays at CareFirst in Lake Havasu City (928)208-4598 if you want to be a CareFirst new primary care patient, understanding that I am not a manager or owner there and CareFirst policies and pricing applies, which I have no control over. The staff there, Megan, Courtney, and Serina are wonderful and I recommend them very highly, and they are contracted with nearly all insurances.

NEW Patient Intake Visit: 1 hour

All office visits are $100

New patient appointments take a great deal of effort and time from us both.

This is very-necessary first-time one-hour visit.

What we’ll do:

A basic full exam while we talk.

Discuss what’s going on, list all of your diagnoses in your chart, and build a list of any other current medical providers, and build a timeline record of your important medical events, surgeries or procedures, and details.

Gather your accurate medication list from your pharmacies, record your medical history in your new chart, and set our first goals.

Unless you have very recent blood tests, I will probably ask you to go have a new set of labs at LabCorp or Sonora Quest, so that we can safely plan and adjust your care.

Imaging, ECG, and any other tests that are needed will be ordered.

Any referrals needed will be made immediately—no waiting weeks for authorizations. You will leave the visit with a referral to take to a specialist in-hand so that you can find out how use your insurance coverage for specialty care wherever possible.

We’ll send requests for any medical, lab, and pharmacy records needed.

Established Patient Basic Visits

Office visit follow up 30-minutes just $100

We will pick up where we left off and figure out what’s next.

A brief exam while we talk.

I’ll make sure your medications and refills are in order, and that nothing is missing.

We’ll update your chart with any new details.

We’ll set new goals or tackle any new problems.

You are welcome to come see me for these in the office as often as you want or need to. We are not restricted by insurance, and you can buy such a visit whenever you like.

Routine trigger point or joint injections, when indicated, included for established patients.
